The short version

• We do not run advertising trackers, analytics pixels, or third-party marketing tags on this site.
• We do not set cookies to track you.
• We do not sell, rent, or trade your information to anyone, for any purpose.
• The only information we collect is what you type into the appointment request form and send us on purpose.

The appointment request form

Our contact page has a form for requesting an appointment. It asks for your name and phone number, optionally your email, whether you are a new or returning patient, when you would prefer to be seen, and an open field asking what brings you in.

Where it goes

When you submit the form, the information is transmitted over an encrypted connection to a server function we control and is delivered as an email to our front desk. It is not written to a website database, and it is not sent to any advertising network, marketing platform, or lead-generation service.

What we do with it

We use it to call or email you back and get you scheduled. If you become a patient, the relevant information is entered into your patient record, and from that point it is protected health information governed by our Notice of Privacy Practices rather than by this policy.

Please don't send us anything sensitive here

Ordinary email is not a secure medium, and we cannot guarantee the security of information in transit to us. A general description of why you would like to be seen is fine. Please do not use this form to send detailed medical history, insurance identification numbers, or anything else you would not want read by someone other than us. If your situation is urgent, call us at (515) 224-1241 — do not use the form.

Information collected automatically

This website is hosted on Google Firebase Hosting. Like essentially all web hosting, it records standard server request information — IP address, browser type, the page requested, and the time of the request — which is used for delivering the site securely and diagnosing problems. We do not use this information to build a profile of you or to target advertising.

Our pages load typefaces from Google Fonts, which means your browser makes a request to Google's servers when the page loads. That request includes your IP address. We do not control what Google does with it; you can read Google's privacy policy at policies.google.com/privacy.

Links to other sites

This site links to places we do not operate, including our Google Business listing and our Facebook page. Once you follow a link off this site, this policy no longer applies and the other site's practices govern.

Children

We treat children as patients, and we welcome them. But this website is not directed at children, and we do not knowingly collect information online from anyone under 13. A parent or guardian should submit the appointment request form on a child's behalf.

Changes to this policy

If we change this policy, we will post the revised version here and update the date at the top of the page.
